Letters and medical reports

If you need a letter or a report completing by a doctor, this is normally not available on the NHS. We undertake a small number of private medical reports for a set fee. Examples include:
Insurance reports
Firearms certificates.
DVLA forms

Fit to letters:

Generally speaking, our doctors are not able to complete certificates to state that patients are ‘fit to’ undertake activities (e.g. gym memberships, sporting events, parachute jumps, expeditions etc). Such requests are legally very different to requests to confirm a patient’s medical history or statements of fact. These requests require the doctor to take medical responsibility for the activity and are often requested by companies to reduce their insurance costs.  This is not an NHS service. For these reasons, we do not accept such requests.  We would suggest patients use a private medical certificate service for such forms
