Patients with certain chronic diseases will be offered an annual review with our nursing team. Patients will normally be contacted in the month of their birthday for their annual review. Patients with difficulties with their chronic disease management (e.g. poor asthma control or hypertension) can also book a follow up appointment with the nurse to review their treatment plan.

Patients with the following conditions will be seen by our chronic disease nurses annually. 

• Diabetes / Pre-diabetes
• Hypertension
• Heart failure
• Asthma
• COPD
• Atrial fibrillation
• Learning difficulties
• Stroke and TIA
• Certain mental health conditions
